Key Responsibilities:

• Conduct assessments and develop person-centered plans that focus on individual strengths and goals
• Coordinate services and supports for individuals with intellectual and developmental disabilities
• Monitor and evaluate the implementation of individualized care plans
• Provide training and support to direct care staff
• Ensure compliance with state and federal regulations

Qualifications:

• Bachelor's degree in Human Services or related field
• Certification as a QIDP (will certify if necessary)
• Strong organizational skills
• Excellent communication and interpersonal abilities
• Ability to work independently and as part of a team
