Qualified Intellectual Disability Professional (Case Manager)
Join Our Team as a Qualified Intellectual Disability Professional (Case Manager) in Chicago, IL!
Are you passionate about making a positive impact in the lives of individuals with intellectual disabilities? Do you have the skills and drive to advocate for and support those in need? Active Visions is seeking a dedicated Qualified Intellectual Disability Professional to provide support to adults receiving CILA services through Active Visions.
Job Responsibilities:
- Oversee a caseload of 12-15 individuals to ensure they are safe, healthy and an active participant in their community
- Provide case management services for individuals with intellectual disabilities
- Develop individualized Implementation Strategy Plan based on assessments and evaluations
- Coordinate and monitor services and supports for each individual
- Advocate for clients' rights and preferences
- Collaborate with a multidisciplinary team to ensure comprehensive care
- Provide crisis intervention and problem-solving support
- Maintain accurate and up-to-date documentation
- Supervise Direct Support Professionals working in CILA
Qualifications:
- Bachelor's degree in social work, psychology, human services, or related field
- At least one year providing direct services to individuals with intellectual disabilities
- Knowledge of relevant regulations and government benefits
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Passion for advocating for vulnerable populations
