Active Visions Inc. has been serving clients since 1994, and continues to grow. We have CILA (Community Integrated Living Arrangement) homes through the northern portion of the city of Chicago as well as some north suburbs. We have small home settings, which allows our team members to focus speicifc attention on each client during their shift. We also serve home-based families through the Chicago area that are looking for individual support for their family member. 

We are looking for a QIDP to act as a service facilitator for these individuals. As a QIDP, you are in charge of oversight in the home, leading the staff to ensure your clients needs are being met, an ensuring all clients have the proper care. You will act as their advocate to the their families, in the community, and at home. It is important that we have team members that understand the rights of our individuals, and fight to protect them.
